Driver Education Instructor Application Attachments

A copy of all of the following documents must be attached to your Driver Education Instructor Application.  No application will be considered complete or will be processed until all of the documents are received by the Department.

  • A copy of original social security card (19-122-160b2)
  • A certified copy of the applicant's birth certificate (19-122-160b3)
  • A copy of the applicant's driver's license (19-122-160b4a)
  • A copy of the applicant's high school diploma or equivalent (19-122-160b4b)
  • A copy of the applicant's driver education instructor completion certificate (19-122-160b5)
  • An original traffic abstract (19-122-160b5)
  • Certificate of commercial general liability insurance (19-122-160b6)
  • A certified or sealed copy of a Hawaii criminal history record check (19-122-160b7)
  • Proof of application for a FBI criminal history record check as defined in Chapter 19-122, Subchapter 4, Hawaii Administrative Rules (19-122-160-b7)
  • Two 2"x2" photos (19-122-160b11)
  • Two Driver Education Instructor Signature Cards
  • State Tax Clearance (19-122-160-b10)
  • Classroom Curriculum (if planning to teach this course) (19-122-250)
  • Behind-the-Wheel Curriculum (if planning to teach this course) (19-122-250)
  • Copy of current motor vehicle registration (19-122-166-1)
  • Copy of current motor vehicle safety check (19-122-166-1)
  • Verification of dual brake in Behind-the-Wheel instruction vehicle (19-122-166-3)
  • Verification of instructor's eye mirror (19-122-166-3)
  • Verification of "Student Driver" sign (19-122-166-4)
  • Proof of automobile insurance as described in 19-122-166-5.
