PARKING PERMIT


1. WHERE TO APPLY:

2. APPLICABILITY:

Applicant required to have a permit to park on the shoulders or auxiliary lanes of a restricted section of highway under the State's jurisdiction.


3. PERMIT REQUIREMENTS:

(1) Provide the following information:


a. Applicant's Name Address and Telephone Number (organization, if any).

b. Location on the highway, from one cross road or address to another.

c. Date and Time of the occasion.

d. No fee required.


4. REVIEW PROCEDURE:

There is a minimum seven (7) working days processing time prior to the date of the occasion for applications made by telephone. If less than seven (7) working days, apply at the Oahu District Office in person. Permit usually takes one (1) day to process.

 

USE AND OCCUPANCY PERMIT
(Lane Use Permit For Construction Work)

2. APPLICABILITY:

Applicant needs a special permit to park on a restricted section of highway under State jurisdiction for construction adjacent to and within the highway right-of-way.

3. PERMIT REQUIREMENTS:

(1) Provide the following information:
a. Applicant's Name, Organization, Address, and Telephone Number.

b. Location in the highway, from cross road to another.

c. Date and Time of construction.

d. Certificate of insurance. Applicant to procure, at his own expense, and keep in force at all times when work permitted under said permit is being done, a policy or policies of public liability and property damage insurance, naming the State as a co-insured, covering the entire work to be done under said permit and for at least the minimum coverage set forth hereinafter.


Bodily Injury or Death:
$100,000 minimum limit, one person
$300,000 minimum limit, each accident

Property Damage:
$50,000 minimum limit, each accident

Applicant will submit a certificate of insurance at the time of application.

e. Non-refundable $10.00 fee.

f. Two (2) set of plans.
 

4. REVIEW PROCEDURE:

There is a minimum seven (7) working days prior to the date of the construction if you wish to apply by phone. Less than seven (7) working days, apply at the Oahu District Office in person. Permit usually takes one (1) day to process.

USE AND OCCUPANCY PERMIT
(Conducting Engineering/Survey, Maintenance And Inspection Work Upon State Highways)


1. WHERE TO APPLY:

2. APPLICABILITY:

Applicant to obtain a permit to perform engineering survey, maintenance and inspection work only - a purely service type of activity. No digging or disturbing of the highway will be permitted under this permit.

3. PERMIT REQUIREMENTS:

(1) Applicant to procure, at his own expense, and keep in force at all times when work permitted under said permit is being done, a policy or policies of public liability and property damage insurance, naming the State as a co-insured, covering the entire work to be done under said permit and for at least the minimum coverage set forth hereinafter.

Bodily Injury or Death:

(2) Submit traffic control plans if work is being done on the roadway.

Permit application can be reviewed and issued on the same day. However, if an inspection of the area has to be conducted prior to the issuance, it may take 1 to 2 days to process.

USE AND OCCUPANCY PERMIT
(Landscaping In Highway Right-Of-Way)

2. APPLICABILITY:

Proposals by others to perform planting, landscaping or landscape maintenance work within the State highway right-of-way.

3. PERMIT REQUIREMENTS:

Submit written request to the appropriate District Engineer and, when new planting or landscaping is involved, two sets of detailed layout plans, drawn to scale, accurately describing the nature and extent of the proposed work.

No fee.


4. REVIEW PROCEDURE:

In most cases, requests are reviewed and processed by the District Office within one (1) day. Projects requiring headquarters review may take three weeks. This may be necessary if the suitability of trees or plants are questionable, there is a possibility of conflict with future highway improvements, integration with future landscaping plans is necessary, other work is also involved, or questionable features exist.

ROUTINE CONSTRUCTION WITHIN A STATE HIGHWAY

2. APPLICABILITY:

(1) Routine construction projects within the State highway right-of-way proposed by others such as:

a. Residential driveways;
b. Utility service connections;
c. Minor and emergency repairs;
d. Minor adjustment of utilities;
e. Overhead and underground utility crossings of other than freeways; f. Construction of concrete sidewalks, asphaltic concrete footpaths and wheel chair ramps; and
g. Soil Borings.

3. PERMIT REQUIREMENTS:

(1) Two sets of construction plans drawn to scale showing the extent and layout of proposed work with all necessary dimensions, clearances and elevations including, but not limited to the following:

a. Location map with north arrow;
b. Location of existing highway facilities such as drainage culverts, signs, sidewalks, traffic signals, light standards, guardrails, etc.; and
c. Traffic control plan.

(2) Insurance.

(3) Fee schedule varies. Minimum fee is $10.00.

(4) Bond.  Minimum is $1,000.00.

(5) Two set of plans.

 

4. REVIEW PROCEDURE:

In most cases, requests are reviewed and processed by the District Office within one (1) day.

(Litter Removal Along A State Highway)

Proposals by others to remove litter from State highway right-of-way.

3. PERMIT REQUIREMENTS:

(1) Applicant shall submit a written request describing the details of the proposed cleanup project and complete an application form.

(2) No fee.

In most cases requests are reviewed and processed by the District Office within one (1) day.

OVERSIZED AND OVERWEIGHT VEHICLES ON STATE HIGHWAYS PERMIT

2. APPLICABILITY:

All proposals to operate or transport oversize and overweight vehicles and loads over State highways.

3. PERMIT REQUIREMENTS:

(1) Application for permit shall be submitted on forms furnished by the Division. Applicant shall provide all information required on the form including sketches showing plan and elevation of wheel arrangements, dimensions, and distribution of load.

(2) Permit fee varies from $5.00 to $25.00.

(3) Certificate of insurance on file.


4. REVIEW PROCEDURE:

In most cases, requests are reviewed and processed by the District Office within one (1) day.

Permit applications requiring headquarters review may take a minimum of three (3) weeks. This may be necessary when the proposed loads are extraordinarily large or heavy.

TEMPORARY USE OF AIRSPACE WITHIN STATE HIGHWAY RIGHT-OF-WAY

2. APPLICABILITY:

The revocable permit is required to control the use of airspace on Federal-aid highway systems for non-highway purposes in accordance with FHWA and State policies relating to management of airspace.

3. REVOCABLE PERMIT REQUIREMENTS:

(1) Application: Forms to be issued by State Highways Division.

(2) Location: Include map and area.

(3) Purpose: Storage and parking. Include type and number of equipment, material and vehicles.

(4) General design for use of airspace: Include plans, maps or sketches.

(5) Duration of use: Month to month revocable permit not to exceed one (1) year unless renewed by DOT.

(6) Revocable Permit fee: Determined by appraisal (fair market rent).


4. REVIEW PROCEDURE:

(1) Receipt of Application and Review of Application for proposed use.
(2) Request of Director's policy decision on proposed use of the proposed area.
(3) Application approved by Director, DOT and FHWA drafting and finalizing of airspace revocable permit incorporating all conditions.
(4) Execution of Revocable Permit.
(5) Executed and approved Revocable Permit forwarded to applicant.

MOTOR CARRIERS - MOTOR VEHICLE MODIFICATION

2. APPLICABILITY:

Hawaii Revised Statutes, Section 286-202, General duties and powers of the director, Subsection 12, states, "To review and approve all plans and specifications for construction in the state or modification of motor carrier vehicles which will at anytime be operated upon the highway."

3. PERMIT REQUIREMENTS (any vehicle over 10,000 GVW):

(1) Application form.

(2) Plans and specifications.

(3) Structural engineer's and welder's certification.


4. REVIEW PROCEDURE:

(1) Satisfaction that the modified vehicle will still comply to the rules and regulations regarding safety. Rules and Regulations to be Observed by Motor Carriers in the State of Hawaii - General Order No. 2.

MOTOR CARRIERS - MOTOR VEHICLE INSPECTION STATION

2. APPLICABILITY:

Hawaii Revised Statutes, Section 286-211. A permit is required to ensure uniformity to the inspection station program.

3. PERMIT REQUIREMENTS:

(1) Application form.

(2) Employment of two (2) certified or registered safety inspectors.

(3) Employment of two (2) certified or registered mechanics.

(4) Certified or registered safety inspector and certified or registered mechanic can be one in the same person.


4. REVIEW PROCEDURE:

(1) Satisfactory inspection of station and equipment.

(2) Availability at station of a certified or registered safety inspector.

(3) Proper liability insurance.
